Coach Clearances / Conviction Report Process
Greetings to our returning and welcome to our new coaches!! Thank you all for your interest in volunteering to coach for TVYFHC.
The TVYFHC Board meets monthly and during one of our meetings we made a plan for updating safety policies for our program. In order to stay in compliance with updated mandates and laws we must maintain clearances and trainings that meet state policies. We hope you understand it is our intention to provide our youth with a safe club to belong to and we feel this is an important part of TVYFHC. In addition to those clearances you must sign a conviction report.
The following are detailed instructions to help streamline your clearance request process. You are required to all clearances to TVYFHC one week prior to the start of practices.
All items must be emailed in digital form to the TVYFHC Board Secretary at TVYFHClub@gmail.com. ***Please put your name in the Subject Line***.
1. PA Child Abuse History Clearance
At this portal, create an account and submit your application online. Once complete, send an electronic copy of the results to the email listed above.

2. Criminal History Access Clearance
Go to the Pennsylvania Access To Criminal History website and apply to conduct the record check online with the Pennsylvania State Police at: https://epatch.state.pa.us/Home.jsp Once complete, send an electronic copy of the results to the email listed above.
3. Arrest/Conviction Report and Certification Form
Complete the conviction report and sign it. Send an electronic copy of the results to the email listed above.
4. If you have not lived in PA during the last 10 years you will also need to complete the FBI fingerprinting clearance.
If this applies to you, this will be a cost.

About Twin Valley Youth Field Hockey Club
The Twin Valley Youth Field Hockey Club was created with the intent to promote the sport of field hockey to youth girls in a competitive, fun, and safe environment where they can learn the basic essentials of the sport.
